Yikes! I sometimes wonder about the contractors who do things like de-icing. Two words: Low Bidder.

Icy conditions forced a Delta regional jet bound for Cincinnati, Ohio, to make an emergency landing at the Indianapolis International Airport this morning.
The pilot returned to Indianapolis shortly after takeoff when cockpit sensors detected ice build-up on the wing flaps at about 8:20 a.m., airport spokeswoman Susan Sullivan said.
The plane landed safely at 8:27 a.m., Sullivan said.

Is it possible they didn't even get de-iced? The snow was decent but it wasn't that hard. Or perhaps they were de-iced and the 10 minute taxi was enough to ice it up again lol.

Yes, I know that. And the pilots do too. I've been on more than one flight where the pilots have to return for de-icing a second time because of too long a hold on the taxiway. Perhaps the issue here was that the pilots weren't aware it had been so long since the first de-icing - perhaps. It may have had nothing to do with that, who knows?
